They have seen the return address, the hospital, the lap, the doctor they saw for 15 minutes. They are not a criminal, they are not reckless, they are not a fool with money, they are sick. That is all. This is not a show that hands you a warm cup of both sides and tell you it is complicated and send you back to bed. There are enough of those. This is a show that walks straight into the machine, put its hands on the gate and tell you exactly how they turn and exactly who they turn on. I build the show for the people who work inside American healthcare and the people who get worked over by it. Same system, different seats, and most days, the same confusions because this industry survived on you not understanding it. So we are going to understand it together up now without apologies.

Today we are talking about medical debt, and I want to be honest with you before we take one more step because honesty is the whole point of the show. I'm not neutral about this. I cannot be nobody who has actually looked at the numbers stay neutral. Roughly 100 million people in this country are carrying medical debt right now. 100 million. That is nearly half the other in America. That is your naval, that is your co-worker, that is the person who cut your hair last week and smile the whole time and say nothing. That is very possibly you.

Charity Care And Hospital Lawsuits

Do you understand what that actually means? That means they pay no federal income tax in more cases, no property tax either. They are handed millions upon millions of dollars in tax breaks by the public, by you every year, and in exchange for those breaks, they make a promise. It is called charity care. The promise is that there will be a safe place for the people who cannot pay. That they will catch the one who falls. That is the entire deal. That is the whole justification for letting a giant profitable institution skip its tasks. So we let you keep the money you owe the rest of us, and you use it to take care of the people who cannot afford wound, who cannot afford care. So, how is that promise being kept? A research team led by a sergeant out of John Hawkent went and actually contacted the lawsuit in the state of Virginia. In a single year, hospitals fired around 20,000 lawsuits against their own patients over on pay build. In one year, and yeah, it's the duty I need you to carry out of this room and never sit down. The non-profit were worse than the for-profit. In that data, the non-profit hospital garnish patients' wages at the highest rate than the for-profit one did. The institution getting the touch break for charity were the most likely to reach into a pocket, was the most likely to reach into a working person paycheck and take a cut off of it before the person even saw it down. One hospital system to that same state sue it on patients, and I know what you are thinking. That is all that was a scandal. Somebody wrote about it. Surely they clean it up. No. This year, this year, reporters in Kingston documented a non-profit hospital garnishing the wages of a mother of two. She thought she'd just been scheduled for fewer hours. That is how quiet they do it. And here is the nerf. She qualified for free care. She was eligible for the very charity the hospital exists to provide. That same hospital reported more than $77 million in charity care that year. $77 million written down on people as charity while garnishing the wages of a woman who was supposed to get that care for free. That is the whole system in one story. So the next time a hospital tells you it is a non-profit, a community institution, a mission-driven place of healing, a safe haven, you ask them one question. Just one. Pay

The Credit Rule That Got Killed

attention here because this is the cleanest window you will ever get into how this system actually serves. For one year, advocate made a simple, obvious argument. Medical debt is different from every other kind of debt. You do not choose it, you do not plan it, you do not want it up. It shows up because your body broke. It does not mean you are irresponsible, it means you got unlucky enough to get sick. So the admin was clean. That single rule could have lifted the weight of that 88 billion dollars in report medical debt of the back of the people carrying it. Millions of people could have washed that credit healed over net, not through some tricks, not through some scheme, but through a simple justice. It was finalized, it was real, it was done. And then in July of 2025, a federal court in Texas stuck it down, erased it, validate the entire rules. Now ask the only questions that matter who went to court to kill it. Not patients, not family, not one single person sitting at a kitchen table with a stock of invisible. It was the credit reporting industry and the lender group. So let me put this as plainly as I know. Yeah, there was a rules that could have helped 100 million people and cost the powerful almost nothing. And the powerful went to court to destroy it, and they won. Now I will be fair because they showed you in the truth and the whole truth. Some protection did survive back in 2023. The major credit bureau on their own agreed to stop reporting paid medical collections and drop the small one on $500 that is still standing, that is real, and it helped real people. But the big rules, the one that could have changed the game for 10 of millions gone dragged into courtroom by the people who profit from your pain. I need you to hold on to what that means because it is the entire thesis of this episode. Proven in front of your eyes. When somebody finally tried to fix a piece of this, the system did not fail to act, the system acted to stop the fix. That is not grip lack, that is not dysfunctions, that is not a well-meaning machine that got stuck. That is a machine working exactly as designed, protecting exactly who it was built to protect, and it was never never built to protect you.

Let me tell you what this depth actually does to a human life because number can go numb on you, and I do not want you numb. I want you clear eyes, and I want you furious and when you're ready. Two out of three in the richest nations in history of the world. The most common road to financial ruin runs straight through a hospitals, and it does something cooler than Ben Rossi's. Go to any other peer nations, the one tracked by the organization for economics, co-cooperations, and development, the wealthier democratic with the same medicine we have, the same science, doctors train on the same textbook, medical bangrosis, is not a category there, it is not a thing that happens to people. A cancer diagnosis in those countries is a medical event, a frightening one, but it is not a financial death sentence on top of the fear. They have sickness too. Everybody has sickness. This was chosen. Every piece of it, the hidden presses, the coupon insurance, the lawsuit, the garnished wages, the credit ruin, the rules they killed. None of it fell out of the sky. None of it is weather. Every single piece is a decision that somebody makes and that somebody profit from, which me and hold on to this because it is the hope buried in the anger, it can be chosen differently by design, not by accident. let me say the quiet part all the way out loud. All of it, medical debt is not a side effect of American healthcare. Medical debt is a revenue strategy. Every dollar you owe is a dollar on somebody's balance sheet. The hospital book it has an asset. The collector buy it for penny on the dollars and then hunt you for the full amount. So let's ask it one more time and let's answer it all the way down. The one questions who benefit? When the press is healing until the surgery, who benefit? The seller who never wanted you to stop in the first place. When the insurer still gets buried, who benefit? The insurers who collected your premium every month and then handed the real risks right back to you at the deductible. When the non-profits sue the poor, who benefit? The institutions that keep the tax break for charity and collect the debt in a way, both. When the rules that could have helped 100 million people get killed in Texas courtroom, who benefit? The lender and the credit bureau who went to keep your sickness on your permanent records and lavish forever. Follow it all the way down, every single time, and you land on the same answer. It is never the patients, it is never the family, it is never the person at the kitchen table, it is never you. The system is not broken. Say it with me and mean it. A broken system feels at its job. This system is succeeding at its job. Its job was never to heal you and let you leave the whole. Be patient, it is not broken, it is built, and what is built by people can be taken apart by people.

Six Moves To Protect Yourself

What do we do about it? Now I told you at the top, I could not leave you angry with nowhere to put it. A promise is a promise. Anger with all a plan is just noise, and I did not come here to make noise. I came here to make you dangerous. And here is exactly what you do. Starting today, starting the second, starting the second you turn this phone off. One never pay a medical bill you have not seen automatic. Never call and demand the land by line, atom by atom bills, not the summary, not the total, the full breakdown, every church name, medical bill are ridden with errors, duplicate churches, churches for things that never happen, churches for a room you were never in. You cannot cash what you cannot see. Make them show you every lance. 2. Ask about financial assistance before you assume you owe anything, and ask again after. Every non-profit hospital is legally required to have a financial assistance policy that is the price of their tax break. But they will not advertise it, they will not offer it to you, you have to demand it in rating or by name. You ask, what is your charity care policy, and I am eligible for it. Remember that mother in cancer, she qualified for free care and got her wages taken away because nobody ever applied for it for her. Do not wait for them to apply it. Make them three Do not put a medical bill on a credit card and run from the medical financial plan they push at you right there in the office. The consumer financial protection bill form that Americans pay around $1 billion in deferred interest on medical credit card in just three years. That is how a build you might have negotiated than in still becoming a debt that compounds against you. Does not grow interest the way a cat does. Do not move it onto something that does. Fourth, negotiate outlaw without one arms of shame. As for the cash press, as for the self-pay rate, ask what the government program could have paid for that exact procedures, an offer to pay that. Remember the master press. List is effections. The number on the bills is the opening bid. It is not the final press. People who ask pay less every single time. So ask five. Get every agreement in writing. Get every agreement in writing and lever in those a quote someone. If you are so silence is the single worst thing you can do. Silence hand then an automatic judgment. And after the judgment comes, your paycheck show up. Response a someone answer is a case you can fight. If someone in law is a wage garnishment, you sound for six. No your state. Protections are pitchwork now. And after the federal rules got killed, your state is where the Fed actually live. Some states have passed their own laws, keeping medical debt of your credit report, capping the interest, forcing hospitals to screen you for charity care before they have even allowed to come after you. Find out what your state has done. Find out what it has not, and then push your people to do more. And here is the last thing, the most important things, the things I need you to walk away with above every statistic I give you tonight. The shame was never yours. They landed you the shame on purpose, so you carry the depth quietly, so you blame yourself instead of them, so you whisper about it alone in the dark instead of shouting about it in the daylight.
